Self care is one of the main building blocks for a patient-centred health service. It’s also a key component of supporting people with long term conditions, improving health outcomes and increase patient satisfaction. Furthermore, by managing patients in the community, supporting and enabling self-care, significant cost saving may be feasible through reducing unplanned visits to hospital or the General Practice.
